Butorphanol is levorphanol with a hydrogen at position 14 of the morphinan skeleton replaced by hydroxy and one hydrogen of the N-methyl group replaced by cyclopropyl. It is a semi-synthetic opioid agonist-antagonist analgesic, used as its (S,S)-tartaric acid salt for moderate to severe pain relief. It acts as an opioid analgesic, mu-opioid receptor agonist, kappa-opioid receptor agonist, and antitussive.
