Buprenorphine is a morphinane alkaloid, specifically 7,8-dihydromorphine 6-O-methyl ether, with a -CH2CH2- bridge connecting positions 6 and 14. The N-methyl group has one hydrogen replaced by a cyclopropyl group, and a hydrogen at position 7 is substituted by a 2-hydroxy-3,3-dimethylbutan-2-yl group. It is a potent opioid analgesic and mu-opioid receptor agonist, as well as a delta- and kappa-opioid receptor antagonist. It is widely used in the treatment of opioid use disorder and increasingly in the management of chronic pain.
